Clients hire their own staff without the assistance of a personal support provider.
Individuals with a brain injury.
Phase 1: Referral & Onboarding
Office Administrator (OA) receives phone call or email. Client Referral Form is completed, and sent to the Case Manager Supervisor (CMS) for review. If client is accepted, a request for client’s initial paperwork is processed along with a basic information sheet. A Client profile is then completed.
CMS schedules initials visit within 24 hours, with meeting conducted within 1 week. Identification of providers and Care Path is completed within 30 days.
Phase 2: Client Services Begins.
Case Manager Supervisor introduces the Client and assigned Case Manager. Case Manager (CM) and Client review Care Path and additional areas of concern.
CM schedules ongoing monthly visit with Client to address needs and review goals. Case Notes for each activity are completed on behalf of the Client.
