Life Skills Coaching

Life Goes On provides community-based case management services to individuals with physical disabilities through the Independent Care Waiver Program (ICWP). 


ICWP is designed for eligible Medicaid members with severe physical disabilities (ages 21 – 64) by providing them the opportunity to live in their own homes or in the community instead of a hospital or nursing home. 


After the intake process is complete, each client is assigned a Case Manager for individualized and specialized ongoing care.

Life Goes On provides individual and small group Life Skills Coaching to  clients under the Money Follows The Person Program in Georgia.


The Life Skills Coaching Team delivers a customized training curriculum based on the results of the individualized needs assessment.  


Coaching topics include managing personal finances, managing health conditions and medications, personal hygiene, home management and more!
